  • 2 valid ID’s (PVAO ID, AFP ID) or any government issued ID's

  • Enlistment or Appointment Order

  • Pictures - 1 pc (2X2) & 2 pcs (1X1)

Associate Members
 

  • AFP Retirees

  • Dependents of regular members within the second degree of consanguinity or affinity

Requirements for Membership:

AFP Retirees​:

  • 2 valid ID’s (PVAO ID, AFP ID) or any government issued ID's

  • Retirement Order

  • Pictures - 1 pc (2X2) & 2 pcs (1X1)

Dependents:

  • 2 valid ID's

  • Birth Certificate

  • Marriage Certificate

  • AFP ID of Principal

  • Enlistment/Appointment/Retirement Order of Principal Member

  • Pictures - 1 pc (2X2) & 2 pcs (1X1)
